Love Handles in Men: Why They Stay and What Actually Works
By Dr. Sushrut Tated, plastic surgeon
Love handles are fat on the sides of the waist, above the hips. Overall fat loss through diet and exercise is the first step, because side crunches alone do not burn the fat over a muscle. For adults near a healthy weight with a stubborn pocket that will not shift, liposuction of the flanks is one option.

Dr. Sushrut explains liposuction surgery
What are love handles?
Love handles are the fat that sits on the sides of the waist, just above the hip bones, called the flanks. In men they are one of the most common places for fat to collect, often along with the lower belly. In gym culture they are the enemy of the “V-taper”, and plenty of young men who train regularly still find they will not shift.
Having love handles is common and not a sign of laziness. Fat distribution is strongly influenced by genes and hormones.
Why are love handles so stubborn?
- Genetics decides where fat goes. Many men store fat on the flanks first and lose it there last.
- Hormones and age. Testosterone falls slowly with age, and stress, poor sleep and alcohol make fat gain around the waist more likely.
- Fat loss is whole-body. When you are in a calorie deficit, the body draws on fat from everywhere, in its own order.
Do side crunches burn love handle fat?
The idea of “spot reduction” is that exercising one muscle burns the fat over it. In a frequently cited trial, six weeks of abdominal exercise alone did not reduce belly fat. A small, more recent trial suggested some local effect with high-volume abdominal endurance work, so the debate is not fully closed. In practice, any local effect is small compared with overall fat loss.
Core training is still worth doing for strength, posture and back health. Just do not expect it alone to remove love handles.
What actually reduces love handles?
- A modest calorie deficit you can sustain, rather than a crash diet.
- Enough protein to protect muscle.
- Strength training for the whole body, plus regular cardio.
- Sleep and less alcohol, both of which affect waist fat.
- Patience. The flanks are often the last area to lean out.
Avoid fat burners, laxatives, starving or “cutting” with diuretics. These can be dangerous and do not target the flanks.
Lifestyle vs non-surgical treatments vs liposuction
| Diet and exercise | Non-surgical fat reduction | Liposuction of the flanks | |
|---|---|---|---|
| Reduces visceral (inner) fat | Yes | No | No |
| Reduces flank fat | Yes, as part of overall loss | Modestly, over several sessions | Yes, in a single procedure |
| Downtime | None | Little | About 1 to 2 weeks of soreness; compression garment |
| Health benefits | Many | None beyond appearance | None beyond appearance |
| Suits | Everyone | Small pockets, people avoiding surgery | Adults near a stable, healthy weight |
| Risks | Low | Swelling, numbness, rarely fat enlargement | Surgical risks: bruising, contour irregularity, seroma, rarely clots |

When is liposuction considered for love handles?
Liposuction is a body-shaping procedure, not a weight-loss treatment. It may be considered when:
- you are an adult at or near a healthy weight, and that weight has been stable;
- a stubborn pocket remains despite a sensible diet and exercise;
- your skin has good elasticity;
- you understand that results depend on keeping your weight stable.
The flanks are often treated together with the abdomen or back to give a smooth waistline. A thin cannula is passed through tiny openings to remove fat, and a compression garment is worn afterwards. Recovery is described in liposuction recovery.
If loose skin rather than fat is the main issue, a tummy tuck may be discussed instead.
Is it love handles, belly fat or loose skin?
These look similar in the mirror but are treated differently:
- Love handles are pinchable fat on the sides of the waist, above the hip bones.
- Belly fat may be under the skin (pinchable) or deep around the organs (visceral fat), which makes the belly firm and round and is linked to diabetes and heart disease.
- Loose skin after weight loss hangs or folds rather than bulging, and does not shrink with more dieting.
A simple waist measurement is useful: a growing waistline is a health signal as well as a cosmetic one, and a doctor may suggest checks such as blood sugar and blood pressure.
Does training still matter after liposuction?
Yes. Liposuction removes a set number of fat cells in one area, but the rest of your body can still gain fat. Regular strength training, cardio and a steady diet keep the result and protect your health. Most people return to light exercise within one to two weeks and to full training after about four to six weeks, as advised by the surgeon.
Myths about love handles
- “You can spot-reduce with the right exercise.” Any local effect is small; overall fat loss matters most.
- “Liposuction is a shortcut to weight loss.” It removes a limited amount of fat to change shape.
- “Fat removed by lipo moves elsewhere.” Removed fat cells are gone, but weight gain can enlarge fat in other areas.
- “Sweat belts melt fat.” They cause water loss, which returns when you drink.
When should you see a doctor?
- Rapid weight gain around the waist with tiredness, stretch marks or other changes, which may need a medical check.
- A firm lump on the flank rather than soft, even fat, which may be a lipoma or something else.
- Weight that will not shift despite real effort over months.
- Worry about your body that leads to skipping meals, overtraining or using unprescribed drugs.
If you feel your body is never lean enough, however much you train, talk to a doctor. The Tele MANAS helpline on 14416 offers free, confidential support in India.
What should you ask at a consultation?
- Is my concern fat, loose skin or both?
- Should I lose more weight first?
- Which areas would you treat together for a natural waistline?
- What are the risks, and how long is recovery?

Frequently asked questions
Can side crunches or twists get rid of love handles?
They strengthen the core but do not burn much of the fat lying over those muscles. Studies of targeted abdominal exercise alone show little or no local fat loss, although results vary between studies.
Why do I still have love handles at a normal weight?
Where your body stores fat is largely genetic. Many men store fat on the flanks and lower belly first and lose it there last.
Do fat burners help?
There is little good evidence that fat-burner supplements reduce flank fat, and some contain stimulants or undeclared ingredients that can raise heart rate and blood pressure.
Will love handles come back after liposuction?
The fat cells removed do not grow back, but remaining fat cells can enlarge if you gain weight. Stable weight keeps the result.
Can liposuction remove belly fat inside the abdomen?
No. Liposuction removes fat under the skin. Fat around the organs (visceral fat), which is linked to health risks, is reduced only by overall weight loss and activity.
What if I have loose skin on my waist after weight loss?
Liposuction relies on skin shrinking back. Loose, hanging skin may need a skin-removing operation such as a tummy tuck instead.
